Planted Private runs on the same values as every Planted membership: integrity, excellence and relationship. That means no testing, imaging, or treatment you don't need, and a doctor who will say so.
Investment
$480 per member per month. Additional home visits are available at $250 per visit.
No long-term contract. Billed monthly.

Sick coverage for visiting family. A parent or loved one staying with you under 30 days is covered too — no scrambling for urgent care.
Priority access. First call on appointments and same-day availability.
OMT, integrated into your visits. Hands-on treatment when it's clinically appropriate, at no separate charge.
An annual exam built around prevention. Full cardiovascular risk stratification, with EKG and labs included.
Lifestyle and nutrition counseling, ongoing and personalized.
Unlimited sick visits and follow-ups.

A note from Dr. Koehn
Planted was built on a simple idea: that primary care should mean an actual relationship with your doctor, not a rushed visit and a bill. Planted Private extends that same relationship as far as it can go with the access, privacy, and continuity a small number of patients have asked me for directly.
This isn't a longer menu of services. It's fewer patients and more time, from a doctor who knows your full picture well enough to tell you honestly when something's worth doing — and just as importantly, when it isn't.
